Team Registration
To register, complete this form and click 'Submit.' All teams registered before August 1, 2009 will have their names posted on the "Online Pledge Form." Also, all individuals and team member registered before August 1 will receive an event T-shirt. Participants will be notified by email when on-line pledges are received.
Registration: Team Captain. Please fill in the form below and click 'Submit' to complete registration. This form is both your individual registration and ‘Team’ registration form. Other team members must fill out the Individual Registration Form. The $100 registration fee is due at time of registration and may be paid using PayPal, by calling and giving a Visa/MasterCard, or by mailing in your check. Please allow 3 days (10 days if mailing a check) after registering your team before having each of your team members complete an individual registration. Required information is marked with an asterisk*.
You will be emailed periodically with information regarding your team’s registration and pledges.
On the day of the event, team captains are required to complete registration for their teams. Please have your team members provide you with any information before coming to the registration table.
