Make Your End-of-Life Care Wishes Known

Completing an Advance Directive is a gift to those you love. Should your spouse, sibling, parent or child ever find themselves in the position to make medical choices for you because you can’t speak for yourself, you’ve let them know what’s most important to you.

Too often when the end of life approaches, critical decisions about health care, finances, relationships, and faith are made in crisis situations.  It is not always easy to think about and discuss medical emergencies and end-of-life plans, but it is also necessary to develop a plan that outlines your desires for future medical needs for these situations when they do arise.

Advance Directives  both living wills and medical powers of attorney  provide a secure and legal method of communicating decisions about personal future medical needs to loved ones and medical providers.

The Maine Hospice Council and Maine Center for End-of-Life Care provides these free resources to help you make these decisions and have access to quality end-of-life care that best meets your needs.
